Our care team takes a patient and family-centered approach to care, respecting patient and family choices, care giving and decision making. Teaching about medicines and medical equipment in the home helps patients and families manage health care needs safely and confidently.

Experienced nurse case managers visits each patient's home. Nurses provide assessment, teaching, medication management and skilled nursing procedures while evaluating progress. The nurse case manager coordinates care with the patient's physician, family and other health care professionals.

Other services can include home health aide visits, social services, and professional therapies such as physical, occupational, and speech therapy. These services will vary depending on your doctor's order.

How is home care paid for?

Medicare, Medicaid, most long term care plans and private insurance plans cover many home care services that your physician has ordered.
